Information shared in session and all relevant materials to your treatment will be held confidential between the client and therapist except for the following situations:
- Written consent for all or identified aspects to be released to a specific person or entity
- If a client threatens or attempts to commit suicide or otherwise conducts himself/herself in a manner that is determined to have substantial risk of incurring serious bodily harm
- If a client threatens grave bodily harm or death to another person, including informing the person identified due to duty to warn
- If the therapist has a reasonable suspicion that a client or other named victim is the perpetrator, observer of, or actual victim of physical, emotional, or sexual abuse and/or neglect of children under the age of 18 years old
- If the therapist has a reasonable suspicion that a client or other named victim is the perpetrator, observer of, or actual victim of physical, emotional, or sexual abuse and/or neglect of elderly adults or disabled individuals
- If a court of law issues a legitimate subpoena for information
- If a client is in therapy by order of a court of law, or if information is obtained for the purpose of rendering an expert’s report to an attorney
- If the patient is under the age of 14 years old, the law may allow parents to examine their medical records if such access is deemed unlikely to be harmful to the child or agree otherwise
